International Evacuation from Agha Khan University Hospital, Nairobi to Wellington Hospital, London
A 64-year-old man who had become paraplegic following tuberculosis spondylodiscitis developed seizures in August 2022 and was admitted to hospital in Nairobi where he underwent spinal fusion. In early 2023 he was readmitted with sepsis, suspected to be caused by an infected bed sore. Surgeons removed the fusion hardware from his spine and noted signs of infection. Following this procedure, he developed a pleural effusion. Drainage improved his respiratory function for only a short time and his condition subsequently deteriorated due to aspiration pneumonia on 28 April 2023. He suffered a short cardiac arrest, was intubated and later a tracheostomy was performed.
We were contacted by the family requesting the patients transfer from Nairobi to London as his situation was deteriorating quickly having been at AKUH for 2 months. Having received his father’s very complex referral, it was clear that he would likely need a prolonged period of ICU admission followed by a significant period of rehabilitation.
The team initially co-ordinated for a private air ambulance from Europe to fly down to Nairobi for the journey to London.
The evacuation involved arranging ambulances to transfer the patient from AKUH to Wilson Airport, and from Stanstead to The Wellington hospital in London; which needed to be seamless due to the condition of the patient.
Again, due to the nature of the patient’s health the MCL team also had to arrange for a specialist anaesthetist and nurse to be on board as well as the correct medical equipment for the duration of the flight.
Crucial to the success of the evac was that the MCL team had to constantly co-ordinate with all three teams of doctors a) in Kenya, b) the team on board the air ambulance and c) the doctors at the Wellington to ensure that all treating medical professionals had the latest updates of the status of the patient, as the patient’s conditions was changing daily.
MCL also helped procure the correct medical visa for patient to allow him to have access to treatment in London.
Once in London to optimally manage the patients care and give them the best chance to recover, our team along with The Wellington Hospital ICU sustained the support of many specialists including a spinal neurosurgeon, a plastic surgeon, microbiology, radiology, neurophysiology and neurology.
HCL were in constant contact with the family throughout the length of the patients treatment in London, to provide peace of mind that the patient was receiving the best treatment possible.
After nearly 6 months of inpatient treatment at the Wellington, having recovered, the patient was discharged and was able to return back to his home country.
